As part of the application process to become a coach,
assistant coach, board member, commissioner, approved volunteer,
and/or any other position of authority within the Mountain Park Athletic
Association (MPAA), I understand and agree to MPAA conducting, or having
a third party conduct, a
national background check on me. I fully understand and hereby
authorize LexisNexus, on behalf of MPAA, to procure a national
background check on me which I understand may include information
regarding my character, general reputation and past criminal history. I
understand that it is a privilege, not a right, to be a volunteer within
the Mountain Park Athletic Association that as part of my participation
as a volunteer, I must pass a background check
in conformity with the background check policy implemented by MPAA.
